  • Physical Examination
  • Background Consent Form
  • Driver's License

START DATES OR CANCEL CLASSES AS DEEMED NECESSARY. PLEASE DO NOT EMAIL OR SUBMIT ORIGINAL DOCUMENTS. PLEASE MAKE COPIES OF YOUR DOCUMENTS AND TURN THEM IN ON THE FIRST DAY. SCHOOL WILL NOT GIVE STUDENTS COPIES OF DOCUMENTS ONCE SUBMITTED. NO EXCEPTIONS !!!
